Patient Encounter Notes Tear Away Pad

The Patient Encounter Notes Tear Away Pad was created to help respiratory therapists stay organized while managing multiple patient assessments, treatments, and follow up encounters throughout a nonstop shift. The front gives you a clear place to keep important respiratory information together and easy to find when it is time to reassess or chart. The back is intentionally left blank for additional notes, patient changes, reminders, or anything else that comes up during your shift. Each pad is made with bright and durable premium paper. The paper is 60 LB, 162 GSM, and 100 percent brightness. I created this pad as an RT who understands our workflow, the constant back and forth, and how important it is to have the information you need handy without searching through scattered notes. This pad helps organize information for subsequent charting but does not replace documentation required by your facility. Always follow your facility policies and HIPAA guidelines. Do not include patient names, medical record numbers, dates of birth, or any other identifiable patient information.
